Bits falling off

A few weeks back, I noticed a suspicious hole in the frame of my bike. Closer inspection found it was a missing allen bolt from the frame.

Today, when I got to work, I thought the headstock looked a little odd. But, being in a rush, I didn't look closely until I got back to my bike on the way home. It turns out there is now a hole there too. I seem to remember a plastic bung there before but I am not sure.

My question is, has anyone else had problems with parts spontaneously loosening and falling off? My bike is garaged overnight and is in a secure car park during the day.

I also lost the rubber bung from the headstock.  I'd had it out looking at how I might mount a SatNav, and while I'm sure I put it back in properly, I noticed later it had gone. As it happens I bought a ram mount a little later so hole filled, but still a little annoying at the time.

Cradley Kawasaki, who I bought it from, were really good about replacing the allen bolt.

I will have to have a word about the plastic bung thing too.

I know the bike is a bit dirty but it is used daily for commuting.
